- Request copies of incident documentation the facility is required to keep (incident report, nursing notes, and related records).
- Write down the timeline while it’s fresh: what time it happened, who was present, what staff said happened, and what changed afterward.
- Preserve identifiers: resident wristbands, room/location details, and any discharge or transfer paperwork.
Avoid common pitfalls:
- Don’t agree to facility statements “for the record” before you understand how records can be used.
- Don’t rely on vague assurances like “it was unavoidable” without reviewing the documentation.
