In the days after a resident falls, the facility will often move quickly—new vitals, transfer to urgent care or the ER, documentation updates, and internal reviews. That can feel reassuring, but it can also mean critical evidence is created, changed, or lost.
In Tempe (and across Arizona), the records that matter most are typically generated inside the facility: incident documentation, shift notes, care plan updates, fall-risk assessments, and medication/treatment logs.
Important: what you say to staff and what you request from the facility early can affect how confidently a claim can be explained later.
