Dehydration and malnutrition negligence often shows up as “small changes” before a crisis. Families may notice:
- Weight dropping faster than expected after a medication change or routine transition
- Dry mouth, reduced urination, or skin that looks less hydrated
- Increased confusion or lethargy, especially after meals
- Frequent UTIs or other infections
- Weakness, falls, or trouble swallowing that appears to worsen
- Care notes that don’t match what family witnesses (for example, family reports that staff didn’t assist with drinking)
These symptoms can also have other medical causes. The key issue is whether the facility recognized risk early, followed care plans, and escalated concerns to medical staff when intake or vital signs declined.
