- Weight loss that seems to accelerate between facility updates
- Confusion, weakness, or sleepiness that appears after a change in routine
- Dry mouth, darker urine, low blood pressure, or frequent falls
- Missed meals or minimal intake that is repeatedly documented as “refused”
- Frequent infections or slower wound healing
Sometimes these symptoms are brushed off as part of aging or illness. The legal issue, however, is whether the facility responded with the level of assessment and follow-up a resident’s condition required.
