- Staff changes, weekend coverage, or a new medication plan can coincide with reduced intake.
- A resident who previously drank fluids reliably may suddenly stop, become lethargic, or develop worsening confusion.

A slow pattern that “doesn’t look alarming yet”
- Intake seems “off” but not dramatic.
- Weight trends down over a couple of months.
- Lab work reflects dehydration risk (kidney strain, abnormal electrolytes), but interventions appear delayed.
In both situations, the key legal issue is not whether the resident had medical challenges—it’s whether the facility responded with the level of monitoring, assistance, and escalation required for that resident.
