Common early warning signs include:
- Rapid weight drop between monthly checks or after a care-plan update
- Dry mouth, darker urine, or reduced urination
- New confusion, drowsiness, or agitation that wasn’t present before
- Frequent infections or slower recovery from minor illnesses
- Falls or dizziness that may be linked to fluid imbalance
- Low intake despite encouragement, especially after dietary changes
If the resident’s condition worsens after a staffing change, a weekend/holiday period, or a transfer to a hospital, that timing can matter legally.
