- Delayed reporting and documentation gaps (common when symptoms show up after a shift or after commuting stress)
- Conflicts in incident accounts—especially when multiple witnesses or overlapping job duties are involved
- Work restrictions that aren’t clearly tied to specific functional limits (e.g., “cannot lift” without specifying weight limits or duration)
- Medical timelines that don’t clearly show the progression from injury → treatment → maximum improvement
A tool can’t determine whether your record reads like a clean medical narrative or like a file with missing links.
