- automated or machine-drafted documentation that omits key details
- imaging interpretation that appears inconsistent with later findings
- clinical decision-support outputs that were relied on without adequate verification
- documentation references to software, analytics, or workflow tools you were never told about
Our job is to translate those record clues into a clear question: Was the care delivered in line with the standard expected of providers in similar circumstances, and did any AI-influenced error contribute to the harm?
