In many Texas hospitals, electronic records can be updated, reformatted, or distributed across systems—especially when care involves referrals, outside imaging, or multiple departments. If you’re trying to understand whether AI tools were used, timing matters.
What we recommend right away:
- Request complete operative and perioperative records (not just the discharge summary).
- Ask for anesthesia and nursing documentation tied to the dates and times of your procedure.
- Preserve imaging and reports (including any addenda or corrections).
- Collect follow-up notes where symptoms first appeared or escalated.
If AI documentation is involved, there may be references that don’t fully explain how the tool was used. Our job is to help identify exactly what to request so the review isn’t based on assumptions.
