- AI-generated or “assisted” summaries in the chart
- automated imaging interpretation language
- documentation that reads like it was drafted from structured data
- decision-support references tied to clinical workflow
- system notes that don’t clearly state whether outputs were verified
Those entries don’t automatically mean negligence. But they can raise safety questions—especially when a complication follows quickly or when follow-up discussions seem inconsistent with the medical record.
