- References to automated imaging interpretation, “decision support,” or software-assisted outputs
- Operative or anesthesia documentation that appears incomplete, delayed, or internally inconsistent
- Reports that describe a step occurring when your recollection, post-op course, or follow-up findings suggest otherwise
