It’s common for charts to include unfamiliar system notes—generated summaries, transcription software language, imaging workflow tags, or decision-support references. In many cases, those entries are harmless. But sometimes they raise safety questions, especially when outcomes don’t match the explanation you were given.
We look for specific record signals such as:
- Documentation that doesn’t align with what happened in the operating room or recovery period
- Mentions of automated imaging or interpretation that weren’t followed by appropriate clinical follow-up
- AI- or software-supported “risk” language that appears to have influenced decisions without clear verification
- Missing details that are unusually basic for the procedure performed
The goal isn’t to blame a tool. The goal is to determine whether the care—around the tool—met the required safety standard.
