It’s common to see unfamiliar wording in operative notes, radiology reports, discharge summaries, or follow-up documentation. Sometimes it’s a harmless workflow label. Other times, it may indicate that an AI-related system contributed to outputs that the care team relied on—such as:
- Automated or software-assisted clinical documentation
- Decision-support suggestions used during pre-op planning or perioperative steps
- AI-influenced summaries or transcription workflow changes
- Imaging interpretation support that wasn’t verified appropriately
If your explanation of what happened doesn’t match your symptoms, imaging timeline, or what later providers told you, that mismatch is often where a legal review begins.
