- Operative or post-op notes mention automated elements you weren’t told about (for example, generated summaries, transcription tools, or software-based assessments).
- Imaging reports and clinical observations diverge, and the delay or misinterpretation may have affected treatment decisions.
- The hospital system’s documentation looks inconsistent across entries—common when multiple platforms, templates, or software workflows are involved.
In cases like these, the goal isn’t to blame a machine. The goal is to determine whether the care team met the standard of care given the tools they used and the supervision they provided.
