People often first notice AI-related concerns in the paperwork. Some examples we commonly see families bring to us include:
- Generated summaries or templated notes that don’t line up with what was actually discussed or performed
- Imaging or report language that suggests automated interpretation was relied on
- Clinical decision-support references in perioperative documentation
- Inconsistencies across records (operative report vs. nursing notes vs. discharge instructions)
- Missing details that appear present in some parts of the chart, but not others
These clues don’t automatically prove negligence. But in a case review, they can help identify where documentation, verification, or supervision may have broken down.
