Medication harm isn’t always a dramatic “wrong pill” moment. More often, it shows up as a pattern your family recognizes before anyone explains it clearly.
Common warning signs to note (with dates/times if possible):
- Sudden sleepiness, heavy sedation, or “can’t stay awake” episodes
- New confusion, agitation, or unusual behavior (beyond the resident’s baseline)
- Unsteady walking, increased fall risk, or injuries after a medication change
- Breathing problems or low responsiveness after sedating medications
- Dizziness, nausea, or symptoms that appear shortly after dose administration
- Symptoms that recur around the same part of the day (morning meds, afternoon meds, etc.)
Even if staff attributes changes to aging, infection, or dementia progression, the timeline matters. Save any discharge papers, hospital after-visit summaries, and any written communication you received from the facility.
