- Sedation that ramps up after dose adjustments, new pain medication, or behavioral meds
- Falls or near-falls following medication changes, especially for residents with a history of balance problems
- Confusion or delirium that appears after starting or increasing drugs that affect the brain
- Breathing trouble, extreme sleepiness, or poor responsiveness after opioid or sedative administration
- Inconsistent behavior from shift to shift that doesn’t match the resident’s baseline
If any of these happened after a medication was started, increased, or combined, the timeline matters. The sooner you document what you observed, the easier it is to connect the dots when records are reviewed.
