In long-term care, medication harm isn’t always dramatic. Families frequently describe changes that seem “small” at first—then escalate over days.
Common warning signs include:
- Sudden sleepiness or difficulty staying awake after dose times
- New confusion, agitation, or delirium that tracks with medication schedules
- Unsteady walking, falls, or near-falls following adjustments to pain, anxiety, or sleep medications
- Breathing issues or slowed responsiveness after sedatives or opioid changes
- Worsening cognition after a transition, dose increase, or medication reconciliation problem
If any of these changes began after an order was started, increased, or combined with another drug, that timing can be critical to the investigation.
