- New or worsening confusion, agitation, or behavior changes after a med adjustment
- Unsteady walking that leads to falls—sometimes following dose changes or schedule updates
- Breathing-related concerns after sedating medicines (including opioids or anxiety/sleep medications)
- Lack of improvement after a “temporary” medication change that continues longer than expected
What makes these cases especially frustrating is that the facility may point to physician orders or routine care—while the timeline of symptoms suggests something went wrong in monitoring, administration, or response.
