Elkhorn is a close-knit community with a lot of family involvement, and that can cut both ways. Families often notice quickly when something doesn’t add up—like a sudden change in mobility after a medication adjustment, repeated near-falls that were never reflected in updated precautions, or a resident being left unassisted when they typically needed support.
At the same time, the care setting may involve multiple shifts and handoffs, and fall documentation can be spread across incident reports, shift notes, care-plan updates, and medical records. When you’re trying to reconstruct what happened, that “paper trail” is where many cases are won or lost.
