- More frequent short staffing during peak demand: transitions between shifts and weekend coverage can impact supervision and safe assistance with transfers.
- Higher risk after medication or routine changes: families often notice dizziness, weakness, or confusion after adjustments—then the facility documents the fall without clearly tying it back to known risk.
- Environmental hazards inside older buildings: uneven flooring, bathroom layout challenges, lighting issues, and worn grab bars are the kind of “small” problems that can lead to major injuries.
