In Henderson and throughout western Kentucky, many families care for residents who spend time in common areas, dining spaces, and hallways that can be affected by real-world conditions—busy shift changes, equipment movement, and high foot traffic around meal times and activities.
When a fall occurs, the facility will usually point to the resident’s medical condition as the reason. Our job is to focus on the more actionable question: What risks were documented before the fall, and what safeguards were (or weren’t) actually carried out afterward?
Key issues we review in Henderson-area cases include:
- Whether the resident’s fall risk assessment matched their actual mobility and balance
- Whether staff followed the care plan during routine transitions (to/from rooms, dining, therapy)
- Whether alarms, supervision levels, and assistance requirements were used consistently
- Whether environmental hazards—like lighting, footwear, clutter, or bathroom safety—were corrected after notice
