In many local cases, we look closely at whether the facility properly responded to changes such as:
- a new medication, dosage change, or medication timing adjustment
- a shift in mobility (walker use, transfers, nighttime bathroom assistance)
- increased confusion, dizziness, or balance issues
- staffing or scheduling changes that affect supervision
- environmental risks—bathroom transfers, wet floors, poor lighting, or unsafe flooring
When the facility says the resident “just slipped” or “wasn’t steady,” we check whether the care plan and fall-prevention steps matched the resident’s documented risk.
