In many Illinois communities, nursing homes may be busy and understaffed, and incident details can get “smoothed over” in later summaries. In practice, families often run into the same pattern:

- Incident reports are incomplete compared to what later medical notes describe.
- Care plan updates don’t match the resident’s mobility needs around the fall.
