- Request the incident report and resident fall-risk documents from the days around the fall.
- Ask how the facility assessed fall risk and whether that assessment was updated after any condition changes.
- Document what you observe now (new pain, mobility limits, fear of walking, confusion, sleep changes).
- Identify where the fall occurred (bathroom, hallway, common area) and whether lighting, flooring, or assistive devices were involved.
- Preserve surveillance if video may exist.
