- Get medical care immediately and ask that injuries be documented clearly in the medical record.
- Request the incident documentation: the fall report, nursing notes for the shift, and any updates to fall risk assessments.
- Ask about response time: how quickly staff found your loved one, whether alarms were triggered, and what steps were taken after the fall.
- Preserve information: if the facility has video coverage of hallways, common areas, or entrances, ask that it be preserved.
- Write down specifics while they’re fresh—location in the facility, time of day, mobility aids used (walker/wheelchair), and what staff said about the cause.
