- Get the incident report (and ask for any addenda or corrections). Don’t rely on a verbal summary.
- Request the resident’s fall risk assessment and care plan that were in place before the fall—then ask what changed afterward.
- Confirm what happened immediately after the fall: how staff responded, whether alarms were checked, and whether the resident was monitored.
- Ask about video preservation if the area is covered by cameras (and request it in writing).
- Keep your own timeline: date/time, where the resident was, what they were doing, who was present, and what was said about the cause.
