- Request your records promptly. Ask for the full chart, imaging/lab reports, discharge summaries, and any referral or follow-up instructions.
- Write down the timeline while it’s fresh. Note symptom onset, visits to urgent care/ER, tests ordered, and when you were told results were normal or “reassuring.”
- Preserve communication. Save portal messages, phone call summaries, and after-visit instructions.
- Don’t rely on memory for gaps. In medical error cases, missing details often become the dispute.
