Before you worry about paperwork, handle safety.
- Get medical guidance right away. Tell the treating provider exactly what medication you believe was wrong and what symptoms you experienced after starting it.
- Preserve the “proof you can’t recreate.” Save:
- the medication bottle(s) and any labels
- pharmacy receipts
- discharge paperwork and medication lists
- any written instructions you were given
- Document the timeline while it’s fresh. Write down dates/times for:
- when you filled the prescription
- when you started taking it
- when symptoms began
- when you contacted the pharmacy or provider
