Medication harms can happen for many reasons. But in Lisle, we commonly see cases where the timeline doesn’t match what a safe medication process should look like.
Look for red flags such as:
- A medication label or bottle that doesn’t match the discharge instructions you were given
- Dosage instructions that changed between the hospital and pharmacy without explanation
- New or worsening symptoms shortly after filling a prescription—especially if clinicians later note the timing
- A pharmacy refill that appears to be the wrong strength, formulation, or intended medication
- Conflicting medication lists across records (for example, what one provider says you should take vs. what the pharmacy dispensed)
If you suspect an error, document what you can immediately. The early records (and what they show) often matter as much as what happened.
