- Who controlled the site safety rules at the time of the incident?
- Whether pedestrians had safe routes (especially near loading docks and delivery traffic)
- Whether training and certification were current and followed for that driver
- Whether maintenance or inspection problems were ignored
- Whether supervisors responded appropriately after safety complaints or near-misses
When an employer moves quickly to manage the narrative, important documentation can disappear—surveillance can be overwritten, incident logs can be “updated,” and witnesses may return to shifts and become harder to reach.
