- Symptoms worsen between visits, and reassessment doesn’t reflect the change
- Records are split across multiple facilities, making it harder to connect the dots
When delays happen, the key question isn’t “was the outcome bad?”—it’s whether the care team met the expected standard given what they knew at the time.
