In a dense urban area, many residents experience fragmented care: an initial visit at a local urgent care, imaging ordered through one facility, lab results handled by another team, and follow-up scheduled (sometimes late) with a specialist. Even when each provider meant well, delays can occur when:
- abnormal results aren’t acted on promptly
- a referral is made, but no one ensures the patient completes the next step
- symptoms are treated as “non-urgent” despite red flags
- follow-up instructions are unclear, or the chart doesn’t reflect they were understood
If you were trying to keep up with work and family demands, the gap between “I was told to follow up” and “the follow-up actually happened” can be the difference between early treatment and a worsened outcome.
