In a community where many patients move between primary care, urgent care, imaging centers, and specialists, diagnostic delays often show up in familiar patterns:
- Abnormal labs or imaging weren’t communicated clearly (or were communicated but follow-up didn’t happen).
- A “watch and wait” plan continued too long after symptoms persisted—common when outpatient visits are spaced out.
- Referral paperwork stalled—for example, a specialist appointment was recommended but not scheduled quickly enough.
- ER visits ended with discharge instructions that weren’t followed by the system, such as missing return precautions, incomplete documentation, or unclear next steps.
If any of those feel close to your experience, your next move is to preserve the chain of events while it’s still available.
