- Abnormal imaging or lab results that weren’t communicated clearly, weren’t acted on promptly, or were followed up too late.
- A first impression that didn’t match the severity of symptoms—followed by repeated visits where the same incomplete workup continued.
- Referral or scheduling breakdowns, where you were told to “follow up” but the timing slipped enough to affect outcomes.
- Care continuity gaps, especially when records arrive late between facilities or when a new provider doesn’t have the full timeline.
