After you notice redness, an open sore, or worsening skin breakdown, act quickly—both medically and legally.
Do this immediately:
- Tell the charge nurse and request a skin assessment be documented right away.
- Ask for the wound staging information (and what stage the ulcer is believed to be).
- Request the care plan and repositioning schedule used for your loved one.
- Get copies of relevant records: wound care notes, skin assessment sheets, progress notes, and care plan updates.
- Write down your observations while they’re fresh: dates/times you raised concerns, what staff said, and what changed afterward.
Why this matters in NJ: records can be created quickly, but they can also become harder to obtain later. Early documentation helps preserve the timeline that matters most in a pressure ulcer claim.
